Care Management is a process to improve patient care by helping patients manage their overall health and conditions and reduce ER/Urgent care visits. This is done by helping you identify health goals and then developing a care plan to meet those goals.
- If you feel your child would benefit from some guidance for behavioral issues related to ADHD or Anxiety
- If you feel your child would benefit from some nutritional counseling
- If you feel your child would benefit from some asthma education
Care Management is a covered service and is available to all patients.
All visits are done here at our office or over the telephone with one of our care managers.

Stuart Rockafellow, PharmD
Stu received his Doctor of Pharmacy degree from the University of Michigan in 2005. He then completed residency through the VA system in Ann Arbor and Albuquerque, New Mexico through 2007. He started as a clinical Pharmacist at the University of Michigan Canton Health Center in 2007 where he remained until 2017. He worked with adults with chronic conditions such as diabetes, hypertension and high cholesterol, and pediatric patients with asthma to help them reach their health goals. He also taught in the UofM PharmD program and Nurse Practitioner programs during this time.
Stu and his wife moved to Traverse City in 2017 and Stu began working for Northern Physicians Organization. He is currently working in 4 clinics with NPO members, three of which are adult clinics. Recently Stu joined Grand Traverse Children’s Clinic. He will be applying his knowledge of medications, motivational interviewing skills, and care management to help children and parents to use their medications in the most effective way possible and to reach their health goals.
